Agency Responsibilities
o To provide detailed accounts in all sections
o No sections must be left blank
o The completion of the form must be accurate and factual
o Ensure all sections are signed and dated
o To attached the following documents with the application:
o Risk Assessment (This must be fully completed and up-to-date)
o Pathway plan
o Support plan
o Financial support plan
o Mental health care plan
o Probation report (if necessary)
o Medical reports and/or plan
o And any other plan you think will support the applicant’s application
Failing To Comply
o If the referring agent fails to comply with anything outlined above, Bethel Homeless Ministry reserves the right to refuse the application and therefore it will not be processed
